Units: About 9,350 Importer: Bayside Furnishings (a division of Whalen™), of San Diego, Calif. Hazard: The lid supports on the toy chests fail to prevent the lid from closing too quickly, posing an entrapment and strangulation hazard to young children. Incidents/Injuries: CPSC received one report of a death involving a 22-month-old boy of Roseville, California. He strangled when the lid of the toy chest fell on the back of his head and entrapped his neck on the edge of the chest of a LaJolla Boat Bed. Description: This recall involves two styles of Bayside youth beds: the LaJolla Boat Bed and the Pirates of the Caribbean Twin Trundle Bed. The preassembled toy chests are designed in the shape of a ship or boat’s “bow” and attached to the beds as a footboard. The LaJolla Boat Bed toy chest has a hardwood top and white wood base with a blue stripe. The Pirates Boat Bed toy chest has a hardwood top, wheel shape and brown wood base with decorative carvings. Sold at: Costco and furniture retail stores nationwide and Costco.com from January 2006 through May 2008 for between $700 and $1,400. RC2 Recalls The First Years™ Children’s Feeding Seats Due to Fall Hazard |
Name of Product: The First Years Newborn-to-Toddler Reclining Feeding Seats Units: About 100,000 Importer/Distributor: RC2 Corp., of Oak Brook, Ill. Hazard: The restraining straps can pull out of the waist strap slots, posing a fall hazard to young children. Incidents/Injuries: RC2 has received 38 reports of straps pulling out or nearly pulling out of waist strap slots. There have been 12 reports of children falling out of the chairs. Description: The First Years Newborn-to-Toddler Reclining Feeding Seat can be adjusted between reclining and upright positions. The seat includes a tray, reversible cushion and a three-point safety belt. One side of the seat cushion has a sewn-in label at the top which says "the first years by Learning Curve." This recall includes all feeding seats except those with an "R" stamped inside a raised circle located on the far left and right of the back of the seatback or those with waist strap slots that are nine inches apart. Starbucks Recalls Coffee Mugs Due To Burn Hazard; Product Manufactured by lab921 |
Name of Product: Starbucks Fusion Coffee Mugs Units: About 167,000 Importer: Starbucks Coffee Co., of Seattle, Wash. Manufacturer: lab921, Seattle, Wash. Hazard: The plastic handle can detach from the body of the mug when filled with hot liquids, posing a minor burn hazard to consumers. Incidents/Injuries: Starbucks has received 23 reports of handles detaching from the mug, including nine that resulted in minor burns. Description: This recall involves two styles of Starbucks 14-ounce Fusion Coffee Mugs. The mugs are white and have a black plastic handle, and a stainless steel base. The cups have “Starbucks Coffee” printed on a black stamp or a brown original Starbucks logo stamp. Manufactured in: China Sold at: Mugs with the “Starbucks Coffee” stamp were sold at Starbucks stores nationwide from February 2007 through November 2007 for about $11. Mugs with the original logo stamp were sold only at Starbucks Pike Place store in Seattle, Washington during the same period and for the same price. Remedy: Consumers should immediately stop using the mugs and contact Starbucks for instructions on returning the cups for a full refund. Name of Product: Football Helmet Chin Straps Units: About 235,000 Manufacturer: Nike Inc., of Beaverton, Ore. Hazard: The chin strap’s plastic cup can break as a result of contact, exposing the player to facial or head injuries. Incidents/Injuries: Nike has received 18 reports of the chin strap breaking, including two reports of concussions, two reports of facial lacerations requiring stitches, and a report of a broken nose. Description: The recalled football helmet chin strap consists of a plastic cup with a foam liner, straps and four metal snaps. They were sold in both youth and adult sizes and come in black/gray and white/gray color combinations. The Nike Swoosh trademark is printed on the outside of the chin cup and DRI-FIT™ is printed on the foam liner. “Made in China” and one of the following four style numbers is printed on the inside of the strap: FA0016 046, FA0016 130, FA0021 046, or FA0021 130. Consumers should stop using recalled products immediately unless otherwise instructed. Name of Product: Beco Baby Butterfly Carriers Units: About 2,000 Manufacturer: Beco Baby Carrier Inc., of Newport Beach, Calif. Hazard: The buckles on the carrier shoulder straps can unexpectedly release tension, causing the strap to slip through, posing a fall hazard to the baby. Incidents/Injuries: Beco has received eight reports of the carrier straps slipping through the shoulder buckles. No injuries have been reported. Description: The recalled infant carriers were sold under brand name “Beco Baby Carrier Butterfly.” The carriers have a black label with a green “b” logo on the left side and a butterfly on the right side. The label is sewn on the outside of the carrier. The following carrier styles are included in the recall: Mia, Pony Express, Carnival, Addison, Ethan, Sophia, Cameron, Ava and Joshua. Name of Product: MagnaMan Magnetic Toy Figures Units: About 1.3 million Distributor: MEGA Brands America Inc., of Livingston, N.J. Hazard: Small magnets inside the figures can detach. Magnets found by young children can be swallowed or aspirated. If more than one magnet is swallowed, the magnets can attract each other and cause intestinal perforations or blockages, which can be fatal. Incidents/Injuries: MEGA Brands and CPSC have received 25 reports of magnets coming loose from the figures. No injuries have been reported. Description: The recalled MagnaMan magnetic toys are figures that have body parts that attach with magnets. They come in either a “Futuristic Warrior” or “Ancient Warrior” theme. Wal-Mart Recalls Charm Key Chains Due to Risk of Lead Exposure |
 WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.
Name of Product: "Hip Charm" Key Chains Units: About 12,000 Distributor: Wal-Mart Stores Inc., of Bentonville, Ark. Importer: FGX International Inc., of Smithfield, R.I. Hazard: The charms on the key chain can contain high levels of lead, which is toxic if ingested and can cause adverse health effects. Incidents/Injuries: The Illinois Attorney General informed Wal-Mart and CPSC on April 16, 2008, that the key chain was found in the home of a 9-month-old child who was discovered to have high blood-levels of lead. The child was observed mouthing this key chain. Description: The recalled key chain has several charms including a button, clover, leaf, heart and a sand dollar. The charms hang from a silver-colored chain. The words “Hip charm” and UPC (#31568 11017) are printed on the product’s packaging. Sold at: Wal-Mart stores nationwide from April 2005 through April 2008 for about $6. The Cranium Cadoo game is packaged in a square cardboard box with an orange background. The seven digit lot number is printed under the plastic tray on the bottom half of the box. Sears and Kmart Recall Play Stoves Due to Tip-over Hazard |
 Name of Product: “My First Kenmore” Play Stoves Units: About 17,000 Retailers: Sears, Roebuck and Co. and Kmart Corp., of Hoffman Estates, Ill. Hazard: A metal bracket connecting the door to the stove can cause a tip-over when the door is opened. This poses a risk of injury to young children. Incidents/Injuries: Sears has received one report of the product tipping over, resulting in bruises to a child. Description: The self-assembled, wooden play stove is painted pink with six white knobs and a timer. The dimensions of the stove when assembled are 11 1/2” W x 13 3/4” D x 32 7/8” H. Sold at: Sears and Kmart stores nationwide from September 2007 through November 2007 for about $100. Manufactured in: Taiwan Remedy: Consumers should immediately remove and discard the bracket that connects the door to the stove to eliminate the hazard. Children’s Wooden Storage Rack Recalled By Jetmax International Following Child’s Death |
 WASHINGTON, D.C. - In cooperation with the U.S. Consumer Product Safety Commission (CPSC), Jetmax International Ltd., of Irving, Texas is voluntarily recalling about 36,000 boy’s and girl’s storage racks with canvas totes. The storage rack can tip over, posing an entrapment and suffocation hazard to young children. CPSC has received one report of a death involving an 8-month-old boy who was asphyxiated after he pulled on the storage rack and it fell over on him. The top rail landed on the infant’s neck. No other incidents have been reported. The storage rack is wooden with three levels and nine removable canvas totes. There are wooden handles on each side of the rack. The boy’s storage rack has natural color wood with red, yellow, green, and navy canvas totes. The girl’s storage rack has white colored wood with pink, yellow, lime, and purple canvas totes. Wal-Mart sold the storage rack under the brand “Home Trend Kids 9 Canvas Bin Boy’s and Girl’s Organizers.” They were sold at Wal-Mart stores nationwide from August 2004 through July 2005 and Ollie’s stores nationwide from July 2006 through June 2007 for about $40. Serious Head Injuries Prompt Recall of Bumbo Baby Sitter Seats - New Warnings and Instructions to Be Provided To Consumers |
 Name of Product: Bumbo “Baby Sitter” Seats Manufacturer: Bumbo International, of South Africa Hazard: If the seat is placed on a table, countertop, chair, or other elevated surface, young children can arch their backs, flip out of the Bumbo seat, and fall onto the floor, posing a risk of serious head injuries. Incidents/Injuries: CPSC has received 28 reports of young children falling out of the Bumbo Baby Sitter seat, including three skull fractures, which occurred when children fell out of chairs that had been placed on tables. Description: The bottom of the children’s seat is round and flat with a diameter of about 15 inches. It is constructed of a single piece of molded foam and comes in yellow, blue, purple, pink, aqua, and lime green. The seat has leg holes and seat back that wraps completely around the child. On the front of the seat in raised lettering is the word “Bumbo” with the image of an elephant on top. The bottom of the seat has the following words: “Manufactured by Bumbo South Africa Material: Polyurethane World Patent No. PCT: ZA/1999/00030.” The back of the seat contains the following “WARNING” – “Never use on a raised surface. Never use as a car seat or bath seat. Designed for floor level use only. Never leave your baby unattended as the seat is not designed to be totally restrictive and may not prevent release of your baby in the event of vigorous movement.” Sold by: Target, Wal-Mart, Sears, Toys R Us, Babies R Us, USA Babies and various other toy and children’s stores nationwide, and various online sellers, from August 2003 through October 2007 for about $40. Manufactured in: South Africa Remedy: Consumers should never use the infant seat on a table, countertop, chair, or other elevated surface.
